After you call our number to request an appointment, you will be asked to text us your email address so that we might send you preliminary documents prior to your first appointment. Also, take a picture of both sides of each of your health insurance cards and either text or email it to us. Shortly after, you will receive a client information form and other documents for you to return to the office at your earliest convenience prior to your appointment.
